YAVAPAI-APACHE NATION — The Yavapai-Apache Nation Alcohol & Substance Abuse Program is seeking community input through a Community Needs Assessment Survey.

The assessment is intended to help the program understand current community needs, concerns and priorities. The results will help guide future prevention, education, treatment, recovery, outreach and support efforts.

The survey takes approximately 7–10 minutes to complete. Responses are requested by Friday, Sept. 18, 2026. Community members can access the online survey through the Nation’s notice.

  • DSS Building — Middle Verde
  • Tribal Housing — Tunlii
  • Community Development — Camp Verde
  • Senior Center — Clarkdale

Completed paper surveys may be returned according to the submission instructions included with the survey.

Fillable electronic copies may also be submitted by email to Glory Lewis at [email protected].

The notice says community participation will help ensure future efforts reflect community needs and priorities.
