If you do not have health coverage
Apply through Health-e-Arizona Plus. The same application can screen your household for AHCCCS health coverage, Nutrition Assistance and Cash Assistance. You can also call AHCCCS at 1-800-654-8713.
If you are American Indian or Alaska Native
If you are enrolled with the Yavapai-Apache Nation, you may be eligible for direct care through the Yavapai-Apache Nation Medical Center and Indian Health Service facilities, including Phoenix Indian Medical Center. Eligible members can generally seek direct care at an IHS facility when it has the staff and services needed. Tribal clinics may have their own registration or eligibility rules, so call before visiting.
IHS and Tribal clinic care is separate from AHCCCS or KidsCare. If you qualify for AHCCCS or KidsCare, you may choose an AHCCCS Complete Care plan or the American Indian Health Program (AIHP). AIHP can be used with Indian Health Service, Tribal 638, urban Indian health facilities and other AHCCCS providers.

Call for local primary care, dental, vision, behavioral health and support services in Camp Verde. Bring proof of enrollment in a federally recognized Tribe.

Phoenix primary care
Call to schedule, change or cancel a primary-care appointment at Phoenix Indian Medical Center. Patients without a primary-care provider may also call.
Prescription refills
Use the Phoenix Indian Medical Center automated refill line for prescriptions that still have refills available.
For Meds by Mail enrollment, call 602-263-1506. If no refills remain, contact your provider.
Phoenix dental clinic
Call for child or adult dental exams. A limited number of same-day adult exams and urgent dental visits may be available.
Other Phoenix hospital services
Call the main switchboard for the emergency department, nurse advice and general hospital questions. For specialty-clinic information, call 602-263-1684.
River People Health Center — Salt River
Call for medical, dental and behavioral-health appointments. Press option 1 for appointments or option 2 for pharmacy. Confirm eligibility and service availability before traveling.
What to have ready
- Tribal ID, Certificate of Indian Blood or other enrollment documentation
- Photo identification and insurance cards, including AHCCCS, Medicare or private insurance
- Current medication list and prescription numbers when calling for refills
- Identity, Arizona residency, household income and employer information when applying for AHCCCS
