Use this checklist when you or your family need support and are not sure where to begin.
Emergency: Call 911 when someone is in immediate danger or needs urgent police, fire or medical help.
Food and essential needs
Ask Social Services or the Food Bank about current food distribution, emergency assistance and eligibility. Bring identification and household information if requested. Confirm dates because supplies and schedules change.
Family, child and safety support
Social Services may assist with child welfare, foster care, parenting support, domestic violence, victim advocacy and family case management. If it is safe to do so, write down what happened, urgent safety concerns and the best confidential way to contact you.
Elders and caregivers
Ask about elder advocacy, senior nutrition, transportation, wellness checks, home-support programs and seasonal assistance. Caregivers should ask whether authorization or medical information is needed.
Health, recovery and wellness
Ask the Medical Center, Community Health Representatives or wellness programs about appointments, transportation, prevention, behavioral-health support and recovery resources. For a life-threatening emergency, call 911.
Housing and utilities
Describe whether you need an application, repair, utility help or emergency accommodation. Ask about eligibility, waitlists, required income documents and after-hours procedures.
School, training and work
Ask education and workforce programs about student support, higher education, scholarships, job search, training and WIOA services. Gather school records, enrollment verification, a résumé and financial-aid documents as applicable.
A simple request checklist
- State the immediate need and any deadline.
- Ask which program handles it.
- Confirm eligibility and required documents.
- Write down the contact name and next step.
- Keep copies and follow up.
